Actually that's a common bug that has been happening recently with different versions of debian based linux, and is obviously related to the boot installation and the firmware.
It's a highly alarming issue, to say the least. It's really bizarre to track down, unless you are actually a debian/ubuntu developer that tests these things. For users, the fix, is to use a different iso for example, or reboot so many times the issue disappears, rather than formally examining the issue competently.
Must of started happening... within the last two months I'd say.
I'd also imagine, as a final aside, that it is a widespread and unreported contemporary issue, like so many things in "our" operating system
